Other states with significant improvement include OR, going from 48th in 2012 to eighth today, Hawaii from 36th to third, Wisconsin from 44th to sixth, and Idaho from 19th to fourth.

Spectrum Health Blodgett, Butterworth, Ludington, United, and Zeeland Community all received A's. The number of hospitals that receive each letter grade is relatively consistent each year, although the latest report showed the smallest number of hospitals who received an F grade.

LeapFrog spokesperson Erica Mobley says the report is created to provide patients with information on how to protect themselves when visiting a hospital. "We are seeing in pockets of the country like Rhode Island, there seems to be a significant effort and achievement to bringing all hospitals to top levels of safety", she said. Virginia Mason Medical Center and University of Washington Medical Center both got "A" grades.

The upshot is that Philly's number of "A" hospitals dropped to five. Washington hospitals ranged from A-D.

The Leapfrog Hospital Safety Grade, which scores more than 2,600 hospitals every fall and spring, ranked Coastal Carolina and Beaufort Memorial hospitals among some of the best hospitals in the country in patient safety. Rhode Island had the most dramatic improvement, jumping from 50th in 2012 to No. 1 in the fall of 2017. It is calculated by top patient safety experts, peer-reviewed and fully transparent.

"Errors and infections in hospitals are the third leading cause of death in America, and people deserve to know which of their hospitals are best at preventing them", said Leah Binder, president and CEO of Leapfrog.

Leapfrog uses data from the Centers for Medicare & Medicaid Services, the Leapfrog Hospital Survey, the Agency for Healthcare Research and Quality, the Centers for Disease Control and Prevention and the American Hospital Association's Annual Survey and Health Information Technology Supplement to rank their hospitals, according to their website. Then, Leapfrog says researchers weigh the evidence, opportunity for improvement and impact of each measure to calculate a grade.

In the Leapfrog study, Bridgeport Hospital ranked below average in 18 out of categories.
